About

The Phyllis Tuckwell Hospice provides a range of support including medical and clinical care, therapies, counselling, social work and practical support for adult patients and families who are living with an advanced or terminal illness (such as cancer, heart, lung or neurological disease). Their services are run by multi-disciplinary teams based at their patient-facing venues and in patients’ own homes.

Key services include:
* Living well programme: designed to help patients living with an advanced or terminal illness, and those closest to them, to manage the impact of their illness, cope with changes, improve wellbeing and remain as independent as possible.
* Hospice at home: provision of care for very ill patients who have chosen to spend their last days, and to die, in the comfort and familiarity of their own home or care home. They find it reassuring to be in their own environment, where they feel comfortable and relaxed.
* In-patient unit: you may be referred to the In-Patient Unit by your GP, District Nursing team, hospital, or by a Phyllis Tuckwell healthcare professional. This may be for assessment, symptom management, pain relief, or for end of life care.
* Carer and family support: If you are a carer, relative, or friend of a patient, you can contact the Patient & Family Support team to receive advice, support or therapies.
* Counselling & Bereavement support: this includes one to one counselling, group support (the Listening Lounge), children and family support, advice and guidance on pre and post bereavement for professionals and families/carers.
* Pastoral care: emotional, social and spiritual support is available via the staff/volunteer team no matter what your spiritual beliefs, supporting those of all faiths or none.

About Phyllis Tuckwell Hospice

Phylis Tuckwell Hospice is the only adult hospice caring for terminally ill people and their families across the whole of West Surrey and part of North East Hampshire both at the hospice and in the community. For many patients, when a cure is not possible, hospice care offers a more dignified and comfortable alternative to spending time in the impersonal environment of a hospital. Hospice care helps patients manage pain and improves the quality of life for the patient and their family. Services include an 18-bed in-patient unit, a day hospice, out-patients unit, and hospice care at home.
