GP Exercise on Referral (Frogmore Leisure Centre)

Everyone Active is working with local GP Surgeries in Hart to promote patient referrals to their Leisure Centres for the Exercise on Referral Scheme. The 12-week scheme will provide supervised aerobic exercise sessions with a progressive programme that is both goal orientated and meets recommended guidelines for the health condition(s) the patient is referred for. The scheme will offer a range of exercise opportunities where possible including gym-based, group exercise, walking schemes and aqua based.

The eligibility criteria to be referred are as follows: currently inactive (ie doing less than 30 minutes moderate activity per week) and have one or more of the following health conditions. • Type 2 diabetes (diet controlled) • Cardiovascular disease • Type 2 diabetes • Type 1 diabetes • Cancer • Obesity • COPD • Dementia • Fibromyalgia or chronic pain • Peripheral Arterial Disease • Depression • Stress• Muscle and joint pain• Anxiety • ME • Hyperlipidaemia • Hypertension • Metabolic syndrome • MS • Osteoarthritis • Osteoporosis • Parkinson’s Disease • Stroke • Asthma • Covid-19 rehabilitation

Referral is by health professional only (eg GP, Practice Nurse, Physiotherapist).

Frogmore Leisure Centre has all the facilities to help keep you and your family healthy, active and happy. The gym is packed with state-of-the-art equipment to help people achieve their fitness goals, whatever they may be. People can also enjoy the outdoor astroturf football pitch that hosts five-a-side leagues and can be hired on an individual basis too and the sports hall hosts a huge range of activities, including badminton, table tennis and basketball. Elsewhere, people can also enjoy hot yoga sessions in their dedicated studio.
